​The best relationships are those built on respect and understanding. The intake packet includes the informed consent and agreement that gives you an understanding of my responsibility as a therapist/ healthcare practitioner and your responsibility as a client.
​
New clients can download and complete the forms required and bring them to your first session. If you do not complete them prior to our session, they need to be completed during our session which may take away from the therapy process getting started.

It should take about 10-15 minutes to complete these forms. So that we can spend your session time focusing on your concerns and not paperwork. Please fill them out ahead of time, before the start of the first session.
